Annual Burning Barrel Permit
There is an annual $10.00 Burning Barrel Inspection Fee. Township Fire Department, Station 2 will call the phone number on the application and set up a time to inspect your burning barrel. Permits are required annually January 1 through December 31.

Bow and Arrow Discharge Permit Application
A permit to discharge a bow and arrow for the purpose of hunting deer can be granted in a no-weapon discharge area if 7 conditions are met (see application) and the applicant has written consent from land owners.

Driveway Permit and Instructions
Municipal Code 4-2.0710 requires anyone installing a driveway to apply for a permit that is approved by the Town Administrator. This permit is $75.00 and needs to include site plan showing parcel size and proposed driveway. Please stop in or call the Administrator.

Meeting Room Application

The Town of Washington permits use of the Meeting/Training room for the purpose of meetings and training sessions to the Town of Washington based organizations and associations, all government entities, and businesses. The Meeting/Training room hours are Monday – Thursday, 8:00 a.m. to 9:00 p.m. and Friday, 8:00 a.m. to 4:30 p.m. All groups using the Meeting/Training room need to sign a permit form and depending on the meeting there may be a meeting room fee. Direct Seller's Permit Application
Municipal Code 3-1.0505 requires that any direct seller to engage in direct sales within the Town needs to register for that purpose.

Dog License Application
Required by Wisconsin Statute 174.05 "the owner of a dog more than 5 months of age on January 1 of any year, or 5 months of age within the license year, shall annually, or on or before the date the dog becomes 5 months of age, pay license tax and obtain a license."

***Must present evidence that all dogs are currently immunized against rabies***

Kennel License Application
In order to qualify for a kennel license, residents must be zoned properly by the Eau Claire County Planning & Development Department.
Kennel license fee is $55.00 for a maximum of 12 dogs. If properly zoned to have more than 12 dogs a fee of $6.00 is assessed for each dog thereafter.

Bartender's License Application
To Qualify:
1. The applicant must present evidence of completion of an alcohol awareness

training program or have held a current license by the Town or a local

community within the last 2 years.
2. The applicant may not have two or more alcohol related convictions or

forfeitures within the previous 12 months, a conviction for selling alcoholic

beverages to minors within the previous 24 months, or a conviction or forfeiture

for possession or selling any illegal drug within the previous 24 months.
3. Subject to Board approval.

Liquor & Beer License Application
Required for all new establishments wanting to sell and/or serve intoxicating liquor and/or fermented malt beverages. Licensing period is July 1 through June 31. Price List

Auxiliary Questionnaire
Must be completed by all individuals referred to on the liquor and/or beer license application. Must be completed by any new individuals listed or referred to when renewing a liquor and/or beer license.

Schedule for Appointment of Agent
Must be completed by all corporations, nonprofit organizations or limited liability companies when applying for the liquor and/or beer license. Must be completed by any new agents when renewing a liquor and/or beer license.

Renew Liquor & Beer License Application
Required for all establishments who the previous year had a liquor and/or beer license. Licensing period is July 1 through June 31.

Temporary Picnic License
Must be used by qualified clubs, churches, or organizations desiring to sell beer, and/or wine at a particular gathering.

Soda Water Beverages License Application
Required for all establishments who sell soda water beverages to be consumed on or off the premises. This includes soda machines. The current fee is $5 per establishment.

Cigarette Permit Application
License is required annually for the sale of tobacco products. Licensing period is July 1 through June 30. The current fee is $25.00 per establishment.

Amusement/Arcade Machine Permit Application
The operator-possessor of amusement machines, meaning any machine or devise coin operated or for the operation of where a fee is changed, must hold a permit. Licensing period is July 1 through June 30. The current fee is $30.00 per machine.

Garbage Hauler's Permit Application
Municipal Code 3-1.0620 requires the collection, transportation, or disposal of solid waste in the Town needs to be licensed. Licensing period is July 1 through June 30. The current fee is $200.00.

Maintain or Repair Utilities within Highway Right-of-Way Permit Application
Municipal Code 4-2.0802. No person may injure of tear up any pavement, sidewalk, crosswalk, ditch, boulevard, drain, or sewer or any part thereof, or dig any hole, ditch or drain in any highway right-of-way, or excavate in or place an obstruction in any such highway right-of-way, or remove any gravel, sod, soil, sand or pavement from any highway right-of-way unless such person has first obtained a permit therefore.

Mobile Home Park Permit Application
Municipal Code 3-1.0410 requires that all mobile home parks license every 2 years. The licensing period is July 1 through June 30.
